this first stream of notes comes mainly from book  mountain beyond mountains  of Tracy Kidder - however what I have selected comes from attempts to understand paul farmer form one chance meeting as well as studying some of his colleagues impacts in a lot of detail Paul had an extraordinary childhood- part of a large family who moved from massachussets to florida where his family often lived out of a trailer bus or a boat His brain/memory was always exceptional He first got interested in Haiti when one of his teenage jobs to earn some money was picking citrus in Florida- a hard working and low paid job more usually done at that time by Haitians He won a scholarship to Duke University to study anthropology. There he met an activist who also represented Haiti workers He determined to spend his first year after graduation in Haiti. From his first experiences in Haiti's lack of health services, he decided that he had to go and study at Harvard Business School. Once he was accepted there, he surprised everyone by communicating form Haiti. Still his grades were among the best. To thiw day he's designed a job around serving Haiti more than half a year and leading the ID (Infectious Disease) school at the Brigham Womens teaching hospital in Boston 'To come. IMPACT ON HAITI - Over 30 years Haiti was , and arguably still is, the poorest nation in western hemisphere- but it has now become a world class partner in training medical staff in serving the most life critical diseases faced by poorest countries and families

FIRST DECADE OUT OF HAITI 1983-1994

Paul Farmer's first year in Haiti started spring 1983 after graduating from Duke - he spent about a year there looking for a base he could grow. Eventually he found Cange - a distant place 3 hours drive out into the country on the bumpiest of tracks especially after Mirebalais. At age 24 he enrolled at Harvard Medical School, and started commuting between Cange and Boston!

Partners In Health (PIH) is founded in Boston by Jim Kim, Ophelia Dahl, Paul Farmer, Todd McCormack, and Thomas J. White to support activities started in Haiti. The purpose to  deliver health care to the residents of Haiti's mountainous Central Plateau region, and later in Artibonite region too.

By 1989, the medical centre in Cange is sufficiently formed to also become a test centre for PIH and its Haitian sister organization, Zanmi Lasante to launch a community-based program for treating tuberculosis. By 1990 Zanmi Lasantein Haiti  launches its comprehensive women's health program, Pwojè Sante Fanm.and in 1993 Paul Farmer wins a MacArthur "genius grant" and uses it to establish the Institute for Health and Social Justice, PIH's research and advocacy arm.Its from 1994 that PIH has enough momentum to make its 2nd development focus Peru with tuberculosis work led by Jim Kim's team.

2nd decade out of HAITI 1994-2004

2000 In Haiti, PIH's sister organization, Zanmi Lasante, initiates a pilot program to provide antiretroviral therapy to 50 AIDS patients. HIV Equity Initiative is one of the first in the world to provide antiretroviral therapy in resource-poor settings. 

.2002 The PIH project in Haiti is one of the first to receive funding from the Global Fund to Fight AIDS, Tuberculosis, and Malaria.

3rd decade out of Haiti 2004-2014

 

2010 PIH and Zanmi Lasante respond to a 7.0 earthquake in Haiti with lifesaving emergency care and a plan to strengthen Haiti's public health system. 2013 PIH opens Hôpital Universitaire de Mirebalais in Haiti. The first medical residents begin training.2012 PIH constructs a public flagship hospital in Mirebalais: a center of medical excellence, a national teaching hospital, and a major investment in Haiti's infrastructure. 

During decade 3 Haiti was hit by a devastating earthquake; it has become famous as an Aid destination (with disadvantages) as well as advantages. The most positive impact for health is that the old teacher training hospital for health having being destroyed in Porto Price, friends of PIH have linked in to build a new teacher training hospital at La Mirebalais. (I was lucky to briefly interview Farmer at a conference in Budapest in 2012 , the opportunities to deliver local health and participate as a world class open learning campus may yet turn out to be Haiti's biggest ever opportunity in 21st C knowledge networking and millennial goal-led economies. Additionally BRAC has made Haiti its gateway to the region. Partners in Health alumn Jim Kim (from summer 2012) leads the world bank. HAITI

PIH was founded in 1987 to deliver health care to the residents of Haiti's mountainous Central Plateau region. In the 25 years since then, PIH has expanded in Haiti's Artibonite and Central Plateau regions, and launched additional projects around the world.

2000 PIH and Harvard Medical School win a $44.7 million Gates Foundation grant to fight drug-resistant TB.

Peru 

1998 Drug-resistant TB patients in Peru complete treatment with cure rates of 80 percent, leading the World Health Organization to revise recommendations for treating drug-resistant TB. 1996 PIH and Socios En Salud launch a community-based treatment program for patients with drug-resistant TB in Carabayllo, Peru. 1994

Boston 1997 PIH's Prevention and Access to Care and Treatment Project (PACT) begins in Boston.. A gift from Thomas J. White establishes the Program in Infectious Disease and Social Change at Harvard Medical School

RWANDA  4.8 Rwanda

2013 PIH inaugurates the Butaro Ambulatory Cancer Center in northern Rwanda.

2011 With the Rwandan government, PIH opens a teaching hospital in Butaro that brings high-quality care and medical education to the entire East Africa region.2005 PIH expands to sub-Saharan Africa, bringing community-based HIV treatment and primary care to two districts in rural Rwanda.

2012 PIH launches a new project in Mexico.

2011 PIH begins support of the COPE program in the Navajo Nation in the southwest United States.

2009 PIH begins the Maternal Mortality Reduction Program in Lesotho, leading to a 350 percent increase in facility-based deliveries.. 2006 PIH launches a new project in Lesotho in southern Africa, bringing primary care and HIV and TB testing and treatment to two remote clinics high in the mountains
